Newcastle Social Prescribing Project Final Evaluation August 2013

In March 2013 ERS and Beacon North Ltd. were commissioned by Involve North East to evaluate the NHS Newcastle West Clinical Commissioning Group (CCG) Social Prescribing Project. This project was one of six localities in England to receive funding from NESTA’s People Powered Health programme.
Prescribing project set out to:

  • Develop a linkwork service within existing voluntary organisations to enable health professionals to refer (vulnerable) people with Long Term Conditions to non-clinical community services and networks;
  • raise awareness and equip health professionals to be able to use the service through roll out of care planning training and support to all local practices; and
  • develop and maintain a comprehensive web-based information resource to support the project.

The project was delivered over a 15 month period and ceased in March 2013.

The Project Manager role was extended beyond the lifetime of the project to 30th June 2013 in order to provide some capacity to coordinate learning from the project and transitional support to ongoing social prescribing activity.

The objective of this report is to provide an objective assessment of the project’s impact and achievements as well as document lessons learned to inform future practice.

Information was gathered from consultation with a range of key stakeholders including: The five VCS organisations who supplied linkworkers to the project , Steering Group members and Working Group members ,The Project Manager , Independent VCS representation
