Primary Care Network Cancer Facilitator (Early Detection)

£35,392 per annum

Funding until March 2025 with possible extension

An exciting opportunity has arisen for a self-motivated, proactive, and efficient individual to join Healthworks to drive improvement in early cancer diagnosis and address cancer inequalities by working with Primary Care Networks (PCN) / GP practices across Northumberland and North Tyneside.

About Us

Healthworks is a national award winning charity working with disadvantaged local communities to tackle health inequalities and improve their health and wellbeing through social prescribing. Based in Newcastle, but working across the region for over 27 years, our patron is Professor Michael Marmot. Our work has been recognised by a King’s Fund/GSK award for our work improving the physical and mental health of people across the North East of England.

This post is the result of an ongoing partnership between Healthworks and Northern Cancer Alliance and local health systems and will be working with a range of key partners across Northumberland and North Tyneside.

Healthworks is a key partner in health inequalities research and designing interventions with our colleagues across the region’s Universities.

The Role

An excellent opportunity has arisen for a Primary Care Network Cancer Facilitator to join our team and work to help make a difference to the health of local communities. You will work in partnership to drive improvement in early cancer diagnosis outcomes, primarily working with Primary Care Networks (PCN) and GP practices across North Tyneside and Northumberland. You will support work to reduce cancer risk, improve uptake of cancer screening, timely patient presentation and referral, and other key early diagnosis work programs. The post holder will be a focal point for early diagnosis activities that will support health professionals.

This role will support, educate and influence primary care teams, health professionals and local health systems to improve cancer services in line with the NHS Long Term Plan, Northern Cancer Alliance plan and the PCN Early Cancer Diagnosis Direct Enhanced Service (DES).

It is envisioned the postholder will have hot desk arrangements to enable them to work effectively across the areas however one day per week will be spent at the head office in Lemington.

You will be a proactive individual with the ability to build and develop strong collaborative relationships with a variety of stakeholders across organisational boundaries to influence change.
