 Ways to Wellness is a new service for people in the west of Newcastle whose daily lives are affected by a long term health condition.

Social prescribing is the use of non-medical interventions to achieve sustained healthy behaviour change and improved self-care. Social prescribing supplements the support a patient gets from health care professionals. A Link Worker works with each person referred, on a one-to-one basis, in the areas they most need support

Patients will be referred directly to the Ways to Wellness programme by their GP. The aim is to improve their ability to manage their illness – through healthier behaviours, increased community participation, and better access to specialist health services.

How can it help?

Ways to Wellness provides non-medical support through specially trained Link Workers, who work with people who are referred by their GP through social prescribing.

They work with you in your local community on the areas where you need help.

How will it benefit?

It will help you build your confidence to make positive choices and improve your ability to talk to your healthcare professionals about managing your condition.

It can be difficult for people with long term health conditions to always feel in control of symptoms, manage changes to their lifestyle and digest all the information they are given by healthcare professionals.

With support from your Ways to Wellness Link Worker, you will develop a realistic action plan to build your skills, knowledge and confidence.  You will start taking control of your health condition, rather than let it control you.

 

Who is it for?

Ways to Wellness is available for people aged 40-74 who have the following long-term health conditions:

  • chronic breathing difficulties (COPS) and asthma
  • diabetes
  • heart disease
  • epilepsy
  • thinning of the bones
  • any of the above with depression and anxiety

and who attend one of the following GP practices:

  • Betts Avenue Medical Group
  • Broadway Medical Group
  • Cruddas Park Surgery
  • Denton Park Medical Group
  • Denton Turret Medical Centre
  • Dilston Medical Centre
  • Fenham Hall Surgery
  • Grainger Medical Group
  • Holmside Medical Group

  • Prospect House Medical Group
  • Roseworth Surgery
  • Scotswood GP Practice
  • Throckley Primary Care Centre
